Job Summary
The LVN Care Coordinator will coordinate patient care, assist with social service needs, perform nursing services, and engage patients in achieving care plan goals within a multidisciplinary healthcare team.
Required Qualifications
- Current Vocational Nurse license in the state of California.
- General computer skills including typing, Microsoft Word, Microsoft Excel, Microsoft Outlook, and medical record data review and entry.
Desired Qualifications
- Bilingual (English/Spanish) preferred.
